This is a unique opportunity to join a Residential Team in providing a combination of Case Management to Community Rehabilitation Service clients as well as Residential Support to other clients in a Group Home at the same location. This Case Management/RSW position works with other members of the team who provide services to adults with severe and persistent mental illness living in a Shalom House program. The Case Management/RSW position will work to help coordinate and collaborate with other providers to ensure that our client's housing, medical, behavioral health, and community needs are being addressed. This will be done through participation in an interdisciplinary team model to develop Treatment Plans with clients and provide a variety of services and support to assist them in meeting their identified goals. The Case Management/RSW position also assists in the Group Home working on client care and teaching coping skills. There are plenty of free training opportunities and a supportive, collaborative team. MHRT1 and CRMA training will be provided.

*Low Case Manager Caseloads 7-10 and position also provides support and skill building to PNMI clients in the same building as a Residential Support Worker

*Hourly rate ($25/hour) is not contingent on meeting productivity expectations

*All clients in one building in Portland

*Shalom pays for all required training including CEU’s

*Mileage reimbursement .58 cents/mile

Associate degree in social services or related field with 1 year of successful community-based mental health work preferred. Degree requirements may be substituted for additional experience if have MHRTC. Must be MHRT-C certified or have provisional certification.

Applicants must have a valid Maine driver’s license with acceptable driving history. We do criminal background checks and driving history checks on all potential new hires.
